A free shipping promo was running for domestic orders. International orders started going out on express courier with nothing charged to the customer.
02 · the cause
A third party discount app was stacking on top of the shipping rate with no country restriction. Nothing in Shopify's own discount settings showed it.

Do this yourself, today
- Turn off discount combinations in your native Shopify settings
- Put a country restriction on any shipping discount you run
- Check the courier cost against what the customer paid on your last ten international orders
